Service Co-ordinator (Part-time, FTC) Leicestershire (Home/Hospital based) Overview The Home from Hospital service helps people reconnect with their community or by engaging in activities of interest. The Service aims to reduce recovery time following a hospital stay, reduce readmissions as well as the overall impact of isolation and loneliness on wellbeing. We are currently looking for a Service Co-ordinator to join us on a part-time, fixed term contract until the 31st March 2023, working 17.5 hours per week. The Role As a Service Co-ordinator, you will be responsible for the co-ordination, delivery and promotion of our Home from Hospital Service across the UHL hospitals in Leicester. In this role, you will be responsible for: - Liaising with service users to discuss support and attending hospital discharge meetings - Liaise with hospital staff to support pre-discharge requests - Maintain accurate, and up-to-date records - Training, supporting and motivating our volunteers - Plan and deliver events to raise awareness of our services within the three UHL Hospitals - Ensuring compliance with associated statutory standards, policies and procedures - Completing administrative duties including data entry and replying to correspondence Working Hours This is a part-time role for 17.5 hours per week, with some flexibility over when these hours are worked. What you ll need - Experience of recording data, monitoring and evaluation processes and maintaining databases - The ability to provide appropriate support and supervision to volunteers - The ability to produce engaging presentations and training sessions - The ability to produce accurate reports - Proficiency in using standard IT systems including MS Office - Excellent communication skills in a range of settings - Excellent interpersonal skills - A full, valid driving licence Ideally, you will have experience of local services and the issues affecting older people and experience of working with volunteers.
